From the School Nurse
To the Parents/Guardians of USD 382 students,
The school nurse helps to ensure that a student’s health permits him/her to function well in school. This may be in the form of preventative counseling,monitoring of illness or interventions, as needed. The school nurse monitors students immunization status to stay compliant with state regulations and will help you find resources to medical care for injury or illness. PLEASE alert the school to any ongoing condition that your student may have, (i.e. diabetes, asthma, seizures, hypoglycemia, cystic fibrosis, pregnancy, heart disorder, ADD, ADHD, or any life threatening allergies). Any medical condition or information should be written on the Student Health History, in the enrollment packet. Please update this yearly. This remains confidential.
Medication
*We want the
students to be independent and safe. However, there cannot be any
type of medication (i.e. over the counter, prescription, syringes,
needles/sharps) in the school. These are all to be kept in the
nurse's office. These items can be misused or be
misinterpreted. There are a few exceptions to this guideline. State
law has provided that students with asthma or anaphylactic
reactions, requiring frequent medication, may carry this medication
on their bodies IF the proper forms are completed and signed.
If your child is in need of this medication at school please
contact the school nurse to obtain these separate forms.
*If a student needs to take medication at school or during a school function, the parent or a physician must complete a "Medication Consent Form". These forms may be obtained in the office. Any medication sent to school has to be labeled for the age of your child, in its original container with a full description on it and will be kept in a locked cabinet.
Immunizations
The State of
Kansas now requires that all students entering K-5, for the
2008-2009 school year, have received 1 dose of the chicken pox
vaccine. If your child has received this, please send a copy of the
immunization record to the school. If your child still needs this,
please contact your family physician or the Pratt County Health
Department to have this done before starting school in the
fall.
These measures may seem extreme, however, our first priority is the safety of every student. Consistent use of these guidelines will ensure good health and safety.Please feel free to contact me if you have any questions.
