P.E. Uniform Order Form

PHS students who are enrolled in P.E. I; Lifetime Sports or Weight Training, are required to have a uniform. Please complete and return this order form -with payment, by August 8, 2008. Please print the student’s name clearly, as names will be put on the shirts and we do not want to misspell anyone’s name.
In order for us to have the uniforms available by the start of school ALL orders, with payment, must be received in the PHS office by August 8, 2008. If your student was previously enrolled in a P.E. class and still has a uniform in good condition, they do not need another one.
