Instructions for Online Enrollment

STEP ONE:
Click on
Student Enrollment Form
(from the Online Enrollment Forms Page)
OR
Click on Current
PowerSchool Users Online Enrollment
(from the Online Enrollment Forms Page)
*Must know username and
password to access*
*Ensure you have the right school selected from
the column as different schools use different forms
*A separate form should be filled out for each
additional student enrolling in USD #382.
*Input the student data on the form directly
into the web browser and print it.
*If you would like to print a paper copy of
ANY form and fill it out with a pen/pencil scroll down to
the "PDF Version of Online Enrollment Forms" below the table
and Click on the desired Enrollment Form to download it to your
computer.
*Make sure all the information is correct.
Then, click the "Print" button.
*Some forms will require a
parent/guardian signature and date. Please be sure to sign and date
the forms, we will not except any form without a
signature.
*Some forms listed in the gold section will print many pages. To save paper, scroll down to print the PDF version of each form and fill out by hand.
STEP TWO: Click Enrollment Fees Form
*Provide the following information by inputting the student data on the Form from the Web Browser and printing it. *Make sure all the information is correct. Then, click the "Print" button.
STEP THREE: Continue to work down the column for EVERY form until finished
*e.g. Food Service Form, School & Lunch
Fees Form, Bus Rider Information Form...
*Make sure all the information is correct.
Then, click the "Print" button.
STEP FOUR: Please bring ALL completed and signed student enrollment forms to the prospective school of attendance.
*Keep a copy of the original signed student enrollment forms for your records and proof of enrollment.
