ATTENTION PARENTS!

Many parents did not check the “Consent to Treat” and/or the FERPA regulations for enrollment.  It is crucial that we have these forms checked in case your child needs any kind of emergency treatment. Please email katieann.wilson@usd382.com to open your enrollment portal and update these very important forms. Thank you.

The Online Enrollment is Online.

The enrollment for 2009-2010 school year is all online.  There are a few forms that do not apply to everyone that can be found at the bottom of this page, they include PE uniform, Free/Reduce Lunch, Auto Debit, Laptop Agreement, Supply Lists, and Extra Curricular Activities Guidelines. You will need to print them, fill them out, and then turn them into the office.  Don't Forget to turn in your payment for books and lunches to the school's office along with the printout of the fee sheet.

If you prefer to attend the One-Stop Enrollment Fair it will be held on August 6th from 12:00 p.m. to 7:30 p.m. and August 7th from 7:30 a.m. to 1:30 p.m. at the PHS Commons.

Login to Powerschool to enroll your child for the 2009-2010 school year. 
(You will have a unique username & password for each child)

To enroll your student you must have your username and password for Powerschool for each child you are enrolling.  The username and password can be found on your child's report card. If you did not receive a report card or you can not read the username and password please contact your child's school. 

If your child is new to the school district you will need to print out the New Student Form for each child and turn it into the school's office filled out.  The office will then set you up with a Powerschool account so you can finish enrolling your child.

INSTRUCTIONS FOR COMPLETING THE ONLINE ENROLLMENT PACKET

The online enrollment process consists of completing a series of forms that provides Pratt USD # 382 with information required for each student registered in the district. As you complete a form, you will be returned to the Online Enrollment Packet page. A check mark will appear next to the forms you have completed. Click on the link to the next form until all of the forms in the packet have been completed. One packet must be completed by the parent or legal guardian for each student.  

Please read carefully over the information on the forms and complete the authorizations where indicated and print forms when indicated. The forms, once submitted is considered electronically signed by both parent and student.

In order to avoid problems completing the online enrollment process, please pay close attention to these points.

Two forms within the packet need special instructions: The Primary Household Packet and the Secondary Household Packet Not all families have a Secondary Household due to divorce, separation, or other custodial arrangement. If your family does not have a Secondary Household, leave all the fields blank, but you must still Submit Form; otherwise, your packet will be considered incomplete.
