RANA CHAPTER  OF THE NATIONAL HONOR SOCIETY

STUDENT ACTIVITY INFORMATION FORM

DIRECTIONS: Please complete all sections of the form. Do not be modest. Every bit of information can be used by the faculty council to assist with the selection process. COMPLETION OF THIS FORM DOES NOT GUARANTEE SELECTION.

 

I. ADMINISTRATIVE INFORMATION (PLEASE TYPE):

I  must follow the following

guidelines in order to remain an active member of Pratt Honor Society.

1. Keep grades at 3.6 average. A one-semester grace period is accepted.

2. Come to monthly meetings or leave a written excuse in the office prior to the     meeting. One warning is given.

3. I must participate in at least four service projects each semester.

I. CO-CURRICULAR ACTIVITIES:

List all activities in which you have participated during high school. Include clubs, teams, musical groups, sports, debate, and major accomplishments in each.

 

II. LEADERSHIP POSITIONS:

This includes elected or appointed positions held in the school, community or work relations. These should be positions in which you were directing or motivating others. Examples would be class officer, team captain, newspaper editor, work area manager, or a community leader.

 

III. COMMUNITY ACTIVITIES:

List community activities in which you have participated and note any major accomplishment in each. These should be any activities outside of school in which you participated in the betterment of your community. For example: community organizations, volunteer groups, community work, Boy or Girls Scouts or community art endeavors.

 

IV. WORK EXPERIENCE, RECOGNITION, AND AWARDS:

This would include any work experience paid or volunteer. It would also give you an opportunity to list awards and honors such as honor roll, letters received in sports, student of the month, anything that would support your acceptance into Honor Society.

 

V. ESSAY

This needs to be a typed written essay on why you would like to be a member of National Honor Society.
